| KIT INSTRUCTIONS |
| If you have not purchased a Premier Preservation Kit and are interested in learning about the process of sending your garment to us, these instructions will tell you what to do! It's a very simple process to get your garment to us... |
| If you have just purchased or received your Premier Preservation Kit, follow these instructions to send your garment to us. |

Draw special attention to any soiled area of the garment by attaching the enclosed "DIRTY" stickers. Only use the
Premier Preservation stickers, as other stickers may leave a residue on the garment. Hemlines are given special
attention, so it's not necessary to mark them. |

Complete the Order Form and Residential Alternate Delivery Form. If you haven't already done so, this is your
opportunity to choose our Diamond Upgrade Service and/or request that repairs be performed on your garment. | Learn about our Diamond Upgrade Service |

Place garment, accessories and additional items in the bag provided. Include the white and yellow copies of the
completed Order Form (with credit card information, if applicable), the Residential Alternate Delivery Form, and
your Personalization Form. Close the bag with the twist tie provided. |

On the bottom of the shipping box, fold down both flap 1's, moisten the shipping tape provided, and place on the
box. Place bag in the shipping box. |

Fold down both flap 2's, moisten the tape provided, and place on the box. Next, place the shipping label on
the top of the box. Refer to your shipping label to determine the carrier.
Bring your boxed garment to the nearest drop-off location for that carrier. |

Remove the tracking portion (receipt) of the shipping label included in your kit, and place it on the back of the pink copy of the order form. Keep the pink copy for your records. |

Turn-Around Time
Thank You for choosing Premier Preservation by Jim's Formal Wear! Your garment will
be handled and preserved with the utmost care and sent back to you within 2-10 weeks
from the day we receive it. Priority Service is available; see Order Form for details. |
